Full Description
VitalCheck Wellness, on behalf of Photon Medical Group, is seeking a culturally sensitive and compassionate Casework Supervisor to join our team. Patient care will be provided in-person and virtually at one of NYC Health + Hospitals’ Humanitarian Emergency Response and Relief Centers (HERRCs) making a difference in the lives of migrants.
Responsibilities
- Coordinates case management services for individuals and families.
- Identifies potential family members for needed services.
- Supports family members to assist them with understanding the process to obtain assistance they need.
- Develops Individual Disaster Case Management Plans in accordance with State of NY policies and procedures.
- Works closely with community resources and care coordinators to ensure good communication is maintained.
- Maintains the contract Database in accordance with contract policies and procedures.
- Develops reports regarding level of work achieved on each case.
Requirements
- Associates Degree in social work, psychology, sociology, or other relevant behavioral science.
- 2-3 years of experience in community-based services, social services, immigrant advocacy, community organizing, and/or humanitarian relief, supporting and advocating for immigrants and asylum seekers.
- Excellent presentation skills as well as oral and written communication skills.
- Must be able to multi-task, be detail-oriented, be organized and work effectively in a fast-paced environment.
- Empathy and sensitivity to the needs of diverse patient populations.
- Experience in community-based services, shelters, immigrants; volunteer or mission experience with disaster relief, refugee, humanitarian relief, preferred.
- Empathy and sensitivity to the needs of diverse patient populations.
- Bilingual in Spanish a Plus.
- Strong interpersonal and communication skills.
- Ability to work effectively and independently in a fast-paced and dynamic environment.
- Proficiency with computers and electronic medical records systems.
- Must be able to commute to New York City and Boroughs.
